Scabies


Scabies is quite common in our community . It's always there, especially in crowded area and lower socioeconomic level families. And, it can be transmitted easily , even form a handshake.

And, you do not get this disease because of the hygiene problem or from your pets !

What is scabies ? 

Image result for a scabiesScabies is a parasitic infection. It is caused by a mite (tiny insect) which build burrows on our skin. And, it causes intense itch, most of the time. 

If you are first time being infected by scabies mites, it takes some time  (about 4-6 weeks) for infestation ( laying eggs in the burrows they build)  



What are the signs and symptoms ? 

Itchiness of the skin is the one that trigger you to seek for help. Usually,  the itch is quite intense that may you can't focus on whatever you want to do and also rashes could be seen on your skin ( usually mild ). 

at the initial stage, you may not feel itchy, but once your body develop an allergic reaction to the mites or their wastes, then you feel intense itchiness around the small blisters and patches on the skin. 

It's quite hard to differentiate the rash between scabies and other conditions. What we are trying to look for is the burrows made by the mites. And this is often the important sign for the diagnosis of scabies. 

The other tip when it comes to diagnosis - ask of the household family member have the similar signs and symptoms as well. As mentioned before, it is very infectious. Close contact will highly result in scabies too.

What are the treatments for scabies ? 

Treat WHOLE family if possible.  
    [to prevent re-infestation]

Usually, it can be treated using permethrin. In the market, there are several brands available for scabies such as A-scabies.

1. Have a shower and towel dry your body
2. Apply the lotion to whole body from neck to toe. And, it is recommended to apply a thin film on the scalp, neck , face, and ears (AVOID eyes and mouth)
[ This is because mites can affect your whole body, however it is less common they inhabit regions above your neck , unless for child under 2-years-old or bed-ridden elderly people]
3. Allow it to dry naturally and wash off 12 hours or leaving overnight
4. May repeat application after 7 days

When you apply the medicine, remember NOT to miss the webs between your fingers and toes, underneath your nails  and behind the ears. 

For the itchy skin, may use other products to soothe the itchiness. The itchiness may last up to a week.

What are the advice ? 

Put the setting - HOT water for your washing machine to wash all your clothes, towels and bed sheets

If your children are infested,  their toys need to be washed as well.

Stop scratching your skin ! And , I know this is nearly impossible especially for children. Believe me, I cannot stop myself from scratching if my skin is itchy. 

Nevertheless, same as most of the skin diseases, once you feel itchy, you tend to scratch , when your skin is scratched and injured, the risk of secondary infection increased. 

Hence , to prevent you from scratching, you need something to soothe your itchiness. So, there is the only thing you can do. 


TO  be aware of one point - sometimes patients comes back and tell you the medicine is not working as they still experience itch. You need to explain to them that it may be due to the fact that  your body reaction to the mites need some time to wane off or you are allergic to the medicine.

In this case, talk to a pharmacist or doctor . 

One more thing , adherence to treatment (following the instructions on how to use the medicine) is also very important. Correct use of the medicines is the crucial point to treat scabies effectively.

Anal fissure

You know what . Anal fissure could be awfully painful when you are experiencing one. Let measure this way, if the pain from giving birth is 10 out of 10, then anal fissure could be 9 out of 10. The pain is intense. 

Trust me, you can't even sit or stand. Tears would be welling in your eyes.

What is anal fissure ?


Anal fissures are the injuries ( tears ) around your anus. Most of the time, anal fissures are caused by too much straining when you are passing your bowel motion that lead to tear of the epithelium tissues around your anus. 

Anal fissure could be caused by constipation as well as frequent diarrhoea (chronic diarrhoea e.g. irritable bowel syndromes) . When constipated, the large , hard stool result in the teat. Frequent diarrhoea can also cause abrasion to the anal wall. 

Also, we need to show our greatest appreciation to our mums. When they are giving birth to us, they are risking to endure the pain from anal fissures owing to the pressure on the perineum ( the bottom part of the body). 

Or , it could be your anus is too small or tight which may lead to the tear when large stools pass through. 

It could be sign of certain GI condition such as Crohn's disease or Ulcerative colitis 

One thing to note that - the healing of epithelial tissue around anal area is slow due to the slow blood flow around that area -  which could take up to  6-8 weeks

What are the signs and symptoms ? 

Sharp Pain. And, you know it. It is a bit different from haemorrhoids, in which you usually feel it when you are passing motion and mostly the discomfort or pain doesn't last long. And if it is external, you can feel the haemorrhoid . 

On the other hand, if you are experiencing anal fissure, the pain can linger for hours after your bowel motion. Also, you may notice the presence of fresh blood on your toilet paper. 

How to we treat anal fissure ? 

First thing first, we need to alleviate the pain. It may be quite serious that you could not even focus one whatever you are doing. 

During the acute attack, lactulose could be use to soften the stools and also make sure there is no further damage to the tear around your anal area. 

Topical analgesics also could be used such as Ultraproct ointment ( cinchocain + fluocotolone hexonate + fluocortolone pivalate)  for superficial anal fissure.  (For a maximum of 4-weeks use , unless instructed by your doctor) 

Other topical ointment such as Proctosedyl or Anusol could be your choices too. 

Oral pain killer could be taken to alleviate or control the pain including Paracrtamol or NASAIDs ( ibuprofen or diclofenac ) 


A hot/warm bath can also alleviate your pain. 

 (Talk to a pharmacist  or doctor for some constructive advice)



Therapeutic management of more severe anal fissure (chronic anal fissure) 

 If the anal fissures fail to resolve. ( longer than 3-weeks) 

Topical nitrates or calcium channel blocker (CCB) is the choice for anal fissure. 

For all these topical treatments, it is recommended to use for at least 6-weeks as the healing process of the anal epithelium is slow. 

(a) Topical nitrates 

Function : to reduce the pain and to increase blood flow to anal area to promote healing. 

ADRS - headache 

- glyceryl trinitrate 0.2% ointment ( insert 1.5cm (about the length of fingertip) TDS into the anus. 

(b) Topical CCB 

-Function - to manage anal fissures 

- Second alternative if topical nitrates does not work or is intolerable. 

eg 2% topical diltiazem, BD or TDS 

ADRS :- headache ( less likely as compared to topical nitrate) 

If it still doesn't work , the doctor may consider the use of BOTOX *botulinum toxin) to paralyse the anal muscles and allow it to slowly heal over time. 

Advice

Once you experience anal fissure for the first time, it is likely that you may experience another episodes if care is not given. 

In long term, dietary management should be done. If the anal fissure is caused by hard stools, you need to take more fibre. It does not hurt to eat vegetables ( well, it might  not be your favourites). However, you will definitely be able to eat more vegetables and fruits if you could bare with the pain caused by anal fissure. 

Exercise ( This is an advice for almost all health issues ). Get yourself moving. The best is 30 minutes daily. However, just try your best to fit in your exercise regimen. 

Trust me, you do not want to experience this excruciating pain.

Plantar Wart




What is plantar wart ?

Plantar wart is a epidermal condition caused by virus.

Wart, also known as verrucae, is caused by a virus, commonly known as HPV ( Human Papillomavirus) . Once HPV infects your skin, it causes the skin to grow rapidly which result in thickening of the skin.


Plantar wart can be found at any part of your feet. The virus infect your skin when it's broken or scratched by the surface.  If the plantar wart is at the sole of your feet, it may become painful when you walk. Otherwise, wart is harmless.


Treatment for wart ? 

Wart could be self healing , especially in the children. This means it may disappear after a few weeks or months.

For wart, OTC products such as Duofilm (salicylic acid and lactic acid ) or any other salicylic acid containing products specifically for wart can be used as the treatment.

Instruction for using Duofilm on warts

1. Soak the wart in hot water for at least five minutes ( to soften the wart)

2. Use a pumice stone to remove dead skin in the area of the wart.

3. Dry thoroughly.

(The first three steps could be done for any other products you used as well - the purpose is to remove the dead skin on the wart - remembering that wart cause thickening of yours skin)

4. Apply 2-4 drops of Duofilm directly to the wart ONCE a day using the plastic applicator.

NOTE : each drop should be permitted to dry before the next drop is added.

5. Avoid application of Duofilm to surrounding normal skin

6. Cover treated area with Band-Aid ( or adhesive tape for a plantar's wart)

7. Repeat treatment every 24 hours. If any irritation develops then stop using Duofilm and wait for the irritation to go away.

If it does not get better within 498u hours, or if it is getting worse call your clinician

Normally, improvement can be seen within  2-4 weeks of treatment . maximum effect occurs after 6-12 weeks of use.

FOR EXTERNAL ONLY (not to apply on mucous membrane) .

NOTE Duofilm is highly flammable.

If Duofilm gets in the eyes or on mucous membranes then flush immediately with water for at least 15-minutes.
